Authorization and Guidelines |
This form you will need to sign and date as soon as your vehicle is left with us. Most repairs are covered by some sort of insurance. The form assumes insurance is involved and states our guidelines and liability limitations. We will also supply you with a "Designated Representative" (DR) form. This form is necessary if you wish us to handle negotiations on your behalf with your or another's insurance company. You must sign and date this form also. When you assign DR status to us, you are basically requiring the insurance company to treat us as your representative in terms of your vehcile. The insurance company may still discuss the claim at anytime with you if you wish or you may direct them to us. This form only pertains to the vehicle damage repair. Lastly, you will need to arrange payment for our services. We do not work for any insurance companies. You the customer are responsible for payment to us. The insurance company may be asked to pay us, but technically it is still your money they are routing to us. A "Direct Payment" (DOP) or power of attorney form will also be supplied to you for signing. By completing this form, you are authorizing the insurance company to pay us directly. As the repairs are completed, the payment(s) arrive so that delays are avoided in releasing the vehicle upon completion. It is our policy that no vehicle will be released without payment in full, unless specific arrangements are made. You are responsible for deductibles, depreciation or prior unrelated damage deducted from payment to us on your behalf by the insurer. We will advise you of deductions as we become aware of them. Payment by cash, check or credit card can be arranged at any time prior to completion of repair. Any lienholders' endorsement must be cleared before a vehicle will be released. Completing the process of repair to a vehicle can range into the thousands of dollars. As we assume the responsibility of repairing the vehicle and all the financial obligations during repair, we ask that the repair bill be paid or arrangements to pay it be determined and commited to before the vehicle is released. Once we are clear on our mutual goals and obligations, repairs will commence. A preliminary estimate of the damages will be prepared and submitted to the insurance company representative along with the DR and DOP form(s). |
